Because customer service and excellence are important to Choices For Community Living, we want to recognize employees that consistently exhibit CCL’s high standard of service and respect. Two employees will be honored for their outstanding commitment to our mission to enable persons with disabilities, their families and other individuals in need to achieve a quality of life driven by choice. Categories are:
- Employee of the Year – This category is for those who work directly with the people we support, like Direct Support Professionals, Habilitation Specialists/Team Leaders, and Assistant Residential Managers.
- Manager of the Year – This category includes Coordinators, Assistant Directors, and Program Directors.
Use this form to nominate an outstanding employee who you feel deserves recognition. Deadline to submit a nomination is Friday, March 20. Online nominations only. Winners will be chosen and announced this spring. Each winner will receive a check for $500 in recognition of their outstanding commitment to our culture and values.
